Thick toenails in old age: causes, care & the right tools

Thickened toenails are common in later life and make trimming difficult. With a little preparation, the right technique and tools that grip well, it works safely and cleanly.

Causes: why do nails get thicker?

Nail fungus is the most common assumption – and at the same time the most common mistake. In older age, thickened toenails usually have several causes at once:

  • Age-related changes – nail growth slows down and the nail plate builds up in layers.
  • Repeated micro-trauma – years of knocking against the shoe, sport, knocks to the toe.
  • Pressure in the shoe and toe deformities – hallux valgus, hammer toes, shoes that are too short or too tight.
  • Reduced circulation – nails with a poorer blood supply grow more slowly and become thicker in the process.
  • Fungal infection – possible, but by no means always the cause.

Thickened toenails but no fungus – what is behind it?

If a test at the doctor came back negative or antifungal treatment brought no improvement, what you usually have is a simple thickening of the nail plate – onychauxis in technical terms. It is caused by pressure and age, not by pathogens. Points that help you tell them apart:

  • Texture: With a fungal infection the nail becomes crumbly and breaks apart at the edge. A thickening caused by pressure or age stays firm – just thick and hard.
  • Colour: Fungus usually discolours the nail yellowish-brown and starts at the free edge or the side. A simple thickening looks more evenly yellowish-grey.
  • Spread: Fungus often moves from one nail to the next over time. Pressure-related thickening usually affects exactly the toe that knocks inside the shoe – often the big toe or the little toe.
  • Progress: If the nail slowly flattens again once the pressure is taken off and it is trimmed regularly, that speaks against a fungus.

Only a test gives certainty – a sample taken by a dermatologist or at a foot care practice. The difference matters all the same: a nail without fungus does not need an antifungal, it needs relief from pressure, regular trimming and layer-by-layer filing to reduce thickness. Months of applying lacquer against a fungus that is not there only costs time.

  2. Cut the nail in segments – the tip first, then the sides.
  3. Do not tear or snap it off – controlled cuts are safer.
